New Patient/First Visit

When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We will ask you to complete several forms that will get you acquainted with our office.

For your first visit, we ask adult patients to schedule about an hour with the doctor. The doctor will discuss your dental and medical history, get acquainted with you and your concerns, and take any xrays that need to be updated. If your schedule permits then you can proceed to the hygienist for your dental health visit.
